From the rear of the door, use the 1st & 3rd square holes for the rear arm-rest position or the 2nd & 4th square holes for the forward arm-rest position.
Although I don't have the image uploaded on my computer, the Accessory Installation manual has a template for locating the holes in the inner door panel, for drilling. Ron's suggestion above, is a great solution, if the template isn't available.
