All those vacuum readings are WAAAAAYYYY low! Create an exhaust leak by loosening a pipe at the manifold on one side or the other, and see if the vacuum comes up. It looks like you're getting an exhaust restriction and the modulator isn't allowing the trans to go into 3rd. gear.
