I've had the best luck removing the doors first, then, leaving the hinges on the truck, whack the pins hard w/a BFH after soaking them w/your favorite penetrating oil. Sometimes I'll squirt them daily a couple days before I start. Use a good pin punch, cheap China ones usually won't do. Knock the bushings out w/same punch, tap the new bushings in place. Replace pins, doors etc.
Down here the rust maggots aren't too bad, I've only had to cut apart one hinge like Scott describes above in the dozens of truck hinges I've done.
