Well I found my template. Shoot me an email with your address and I'll send you a cardboard copy.
What I did was make a sample from 1/4" MDF that you can actually bolt to your trans to make sure it works before you make one out of 1/4" aluminum. You will also need to drill and tap two holes in your adaptor plate and make a "L" bracket for the rear mounting point.
