I just measured my 54 with stock plain-Jane rear bumper which is 70" total width and the box width is 54". I also have a 93 S10 with a similar bumper to the one you pointed out. It is 66" total width and box width is about 56". Houston 54 used that same style of bumper on his great looking truck.
It would obviously require some bracket modifications but I think it might be something you could use. Check out his "54 progress pictures" from a few days back in the "Hi-pro" forum. The extra width would still be inside the fenders and yet protect your tail lights.
