Same situation on my 46. Colorado truck, but has never had a heater/defroster in it. The knock outs are there though, exactly where JiMerit said they are.
My Dad jokingly told me one time, "Back then, we were just glad to be out of the wind and wet. We didn't know we needed a heater".
