Yes, popping out of 3rd is either worn input shaft clutch teeth or the synchronizer ring. Grinding from a dead stop going into gear is a clutch issue - not releasing all the way. Could possibly be that the disk is binding on the input shaft splines, or a bad pilot bushing. Or possibly too much free play before the clutch pedal feels harder. That's an adjustment thing.
