Unfortunately the cab structure has failed and will need to be rebuild to have the cab securely attached to the frame and level. My cab was not quite that bad but I needed to replace the floor from the seat forward into the firewall, the inner, outer, and inner to outer cowl panels. As Gray_Ghost says it can be done but it is not easy and if you are not going to do it your self can be pricey.
