The stud for the bellcrank does not get a washer. Just torqued into place. That it appears to be backing out suggests some binding. How is the bellcrank attached to the stud? It should be by a wire clip. Is the stud and bellcrank lubricated? How about the thrpottle linkage and kick down linkage where they attach to the bell crank? Can you post a photo?
