I used T-nuts through the plywood. I believe you can see a hole in the first picture. The bottom pads, I probably just used regular screws through the perimeter tube. As for the foam, you'll need to find a local automotive upholstery supply.
Regular T-nuts. The screw in style I used. As an extra measure to make sure they didn't spin or come out.
