A previous owner "fixed" the shaft on one of my regulators by filing a flat area on it making it into a D shape. The set screw on the handle contacted the new flat area.
The real problem was the handle. It's made of softer material than the shaft so the teeth were worn out. A new handle was the proper fix.
Make sure the problem really the shaft; not the handle.
