Barnfind is on the money with the Ford lug pattern. You'll need '97 model or earlier wheels from a Ford.
The front bumper looks like one for a 1/2 or 3/4 ton truck (and maybe 1 ton). The bigger trucks had a deeper channel bumper.
Looks like my stock rear bumper to me.
