The output shaft of the t-5 will fit any standard chevy yoke. You can grab one off the shelf at a hot rod or racing shop, or you can pull one from the junk yard.
If I were you I'd just go to a driveline shop and tell them what you need. They can make you a drive shaft with u-joints and the front and rear yokes you need for around 100-150, which is probably how much you'll spend trying to cobb together the parts and have them build it using your parts. So, you'll end up spending more time doing it the hard way. Call the driveline shop to find out exactly what they need to know from you and how much it'll cost, I think you'll be surprised.
