You probably have the clutch disc installed backwards. The center hub of the disc has several damper springs in it. The side of the disc where the damper springs stick out the furthest needs to go back inside the pressure plate. If the springs are installed toward the flywheel, they drag on the flywheel attaching bolts and don't let the clutch release fully.
