I agree that the clip has likely failed. Good idea to make sure the pivot ball isn't badly worn. Also, make sure the clutch pedal linkage is properly adjusted. If the fork arm is contacting the bell when the pedal is in the full down position, then that would have enough leverage to pop the fork off the pivot ball.
