I suppose that is more believable than they worked loose.
They were installed with a big hydraulic header. All production equipment needs repair and adjustment from time to time.
Doesn't say much for the in process inspection.
Another possibility was out of spec rivets. If the diameter was under size they wouldn't have tightened correctly. Another inspection problems.
