With all the chains and crap out of the cab do you see a "saging" in the floor? The door sill area at the edge of the floor to the outside should be perfectly flat from the front door post to the rear, is it?
Have you tried to loosen up the bolts on the door at the hinge on the top hinge and lifted the door?
