I have a Clark 285V70 here in CT. It's a 5 Speed in good shape, I think it came out of a Topkick. Fifth gear is 1 to 1 so probably not what you are looking for but thought I would speak up. There's a military surplus place in Philly that my have an exact match of what you described, and some of heir stuff is MIL rebuild or has never been out of the crate. Remember that these transmissions are large and heavy, best in a C40 - C-60.
