Once upon a time a previous owner installed some big truck mirrors. Holes in the door and top of the door above window have been welded shut and doors refinished. I plan to buy and install the correct mirror. I recall , or think I do, that someone posted the "template" that is used to locate the holes for the mirror on the cowl. I looked inside under the dash and it does not look like the "optional" mirror was ever installed. Was there a "backing plate" for that mirror that went on the inside? Advice, suggestions, anyone ?

There is a template/instruction set that came with the big truck mirror kit. You just flip it for the opposite side. There is a plate underneath for the bolts to go through under the cowl on both sides. https://www.flickr.com/photos/advance-design_parts_co/9647482921/in/album-72157631868452754/ If you PM me with your email I can send PDFs of the template and instructions. It works for locating the small truck mirrors as well.
Look closer. There is a support that already has the proper holes punched out that you can use as a locater to drill out the outer sheet metal. You will drill it it out from the inside. You may have to remove glove box to get access to them.
